Varje anrop från Claude, Cursor eller annan MCP-klient loggas: tool_name, scope, status, duration_ms, error_message.
När admin agerar som klient loggas hela sessionen med expires_at, actions_log[], IP, user-agent. Grant-baserad, aldrig JWT.
